Brookline, MA Brookline, MA Part-time Full-time Part-time Full-time $31 - $34 an hour $31 - $34 an hour $5,000 SIGN ON BONUS Conditions apply as per MAB's policy LPN Case Manager for 3 Young Adults Seeking mission driven, creative and motivated individual to provide case management, advocacy and health and wellness support and education for 3 young adults in a new and innovative group home located in an apartment in Brighton.
This program offers a therapeutic setting for young adults to develop the skills they need to successfully live in the community including skills for independent living, self-management, healthcare management, interdependence and self-determination.
This program is a transition program that provides short-term support (3-5 years) for skill development and a scaffolded transition to their next steps.
The LPN provides medical case management, direct nursing care as needed, advocacy and health and wellness education for the medical/preventative health services for 3 young adult participants with complex health needs in a transition focused community-based residence.
QUALIFICATIONS Experience working with young adults Experience working with individuals with psychiatric conditions and/or co-occurring disorders Excellent interpersonal skills; ability to build positive and collaborative relationships with young adults, families, staff and external stakeholders.
Required:
LPN license Ability to read, analyze, and interpret client records, medical reports, professional journals, technical procedures, or governmental regulations.
Ability to write reports, business correspondence, and procedure manuals.
Ability to effectively present information and respond to questions from groups of managers, staff members, clients, other service providers, family members and guardians, and the general public.
Ability to define problems, collect data, establish facts, and draw valid conclusions.
Ability to interpret information from a variety of sources and make appropriate decisions in situations where limited standardization exists.
CERTIFICATES, LICENSES, REGISTRATIONS Current Licensed Practical Nurse (LPN) License Current CPR/First Aid certification Current, valid, US drivers' license for at least one year.
All candidates must be able to successfully pass a CORI, reference, multi-state background and driving record check Job Types:
